The most common failure pattern is buying a copilot and then expecting it to become a workflow system later.
That usually creates four problems:

- no explicit escalation path
- governance retrofitted after the pilot
At that point, the organization has to decide whether to heavily customize the tool, accept shallow usage, or start over with a better operating model.

Where copilots still fit
This is not an argument against copilots. They remain useful for:
